You have to submit the following documents (self attested) to

1. Affidavit (format given behind the form)
2. Birth Proof (10th certificate)
3. Residence Proof (Voter ID)
4. Aadhar Card
5. PAN Card
6. 3 Passport Photos
7. Passport

Other Common documents:

1. Marriage Registration Form
2. Witness Form (must have PAN card)
3. Original Marriage Invitation from Groom's Side
4. Ceremonial photo self attested by Husband and Wife
